Insurance and Payment Options

Please understand that although we are happy to file claims on your behalf as a courtesy, the following should be noted:
- Your dental insurance policy is a contract between you, your employer, and your insurance company. We are not a part of that contract.
- Your treatment plan is individually tailored and is not based on your insurance benefits or lack thereof.
- Not all services are a covered benefit in all contracts. Some insurance companies arbitrarily select certain services they will not cover.
- It is your responsibility to know the parameters of your policy, and what may be covered or excluded. We cannot act as an intermediary with your carrier or employer.

Claim Filing and Payment Process
We can file your dental insurance claim forms as a courtesy.
- Your insurance company's benefit check can be sent directly to our office in special circumstances.
- You are responsible for the deductible and co-insurance portion that will be due at the time of treatment.
- If the Insurance company pays you directly for any balance that you may owe, please forward it immediately to our Cypress Dental Office TX.
- Claims submitted promptly Benefits expected within 30-45 days.
- If your carrier does not clear the claim within 60 days, unpaid portions become "self-pay" and a statement will be issued.
- You are ultimately responsible for all amounts not paid by your insurance company for whatever reason.
- We will be happy to provide a letter with all of the relevant information for your signature and mailing to your insurance company concerning unpaid benefits.
Refund Policy

- Request for a refund on any pre-paid treatment that has not been started is allowable at any time. All refunds shall be made in the original form of payment, except those paid in cash. These shall be returned in check form. Fifteen working days are allowed for processing the request for a refund.
- Any amount paid through third-party lenders should be refunded to the same account from which money was borrowed.
- You will be required to settle all pending claims with your insurance company before any refund is effected.
- All services rendered are final, and once treatment is started, it is non-refundable.
Patient Satisfaction
